LSS is committed to supporting individuals in making their own decisions that shape their quality of life, empowering them to live the life they imagine for themselves. Person-centered support means our wide array of services are specifically designed to meet the unique needs of individuals in a variety of settings.

My Life, My Choices is our innovative service delivery design focused on ensuring people with disabilities have meaningful relationships, access to services and opportunity to contribute to community.

Services for Disability Services

Camp Knutson

A camp for youth and adults with disabilities, medical conditions and identified needs. We partner with other organizations to host camps for youth and adults with identified needs. Situated on a beautiful north-woods chain of lakes, Camp Knutson is also a retreat facility September through May.

Care Coordination Services

Through a partnership with health care plans, care coordinators work with individuals to promote access to services and resources that help them prevent future hospitalizations and stay healthy in their homes.

Employment Services

Employment specialists help people with disabilities find meaningful work based on their individual interests and goals. Support can include skill and resume-building, mock interviews and on-the-job training.

Services that support the financial and legal health of individuals with disabilities.

Host homes family supporting an individual

Residential & In-Home Support

Housing and support options for individuals with disabilities, including traditional supported living homes, family Host Homes/shared living options and specialized housing for individuals with significant behavioral or medical challenges.

Discovering Self-Advocacy

Self-advocacy opportunities and resources for individuals with disabilities. At Lutheran Social Service of Minnesota (LSS), we have put together resources throughout the state of Minnesota to connect self-advocates through regional groups and organizations.
